VCU PMDC PD Caregivers Survey
Hello, caregiver of a Parksinson's Disease loved one,
I’m Carson Spicer, a senior at Maggie L. Walker Governor’s School. As part of my senior mentorship project, I’ve had the privilege of working with Bonnie Mahl, Sr. Community and Engagement & Outreach Coordinator at VCU’s PMDC, to improve the lives of Parkinson’s disease caregivers.
For my senior project, I’m focusing on the challenges caregivers face when caring for a loved one with Parkinson’s disease. The goal is to gather valuable insights through the short survey below to get feedback about the PD Caregiver resources offered by the VCU Parkinson’s Movement Disorders Center(PMDC). As a PD Caregiver, your input will be incredibly helpful in shaping better support for caregivers like you.
Thank you so much for your time and participation. Your responses will contribute to improving caregiver support. If you have any questions or would like more information about the project, please feel free to email pdcenter@vcuhealth.org
Thank you again,
Carson Spicer
